
Healthcare consultancy CCM RPM Help has announced an update to its implementation service, aimed at helping independent pharmacies establish structured Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs. Pharmacies already perform much of the day-to-day engagement that qualifies under CCM and RPM, including medication follow-ups, patient coaching, and coordination with physicians. Despite this, the vast majority of that clinical work goes unreimbursed. According to the Centers for Medicare and Medicaid Services (CMS), roughly two-thirds of all Medicare beneficiaries have two or more chronic conditions, making them eligible for CCM services. Meanwhile, providers enrolled in both CCM and RPM can generate between $140 and $210 per patient per month in combined Medicare reimbursements, based on national average rates from the 2025 Physician Fee Schedule.
CCM RPM Help notes that despite these established reimbursement pathways, many pharmacies remain unaware of their eligibility to participate. The consultancy program addresses this gap directly, walking pharmacy teams through the steps needed to establish compliant workflows, train staff, and secure collaborative agreements with local providers. The firm states that this approach allows pharmacies to receive compensation for care they are already delivering, while strengthening their relationships with both patients and physicians.
